Please make sure we have all the information about your insurance prior to your visit as most insurance plans require prior authorization for services.

Insurance/Payment Policy

In order for your visit to go as smoothly as possible we ask that all necessary insurance information be provided during the time you make an appointment. Your insurance will be accepted according to the benefits outlined by the policy you chose. If you do not have insurance, payment in full is due the date of service. For your convenience, we accept Master Card, Visa, American Express, Discover, Care Credit, cash and checks. As a courteous to our patients we will file insurance claims for all insurance plans we participate in as long as you provide us with complete information.

It is important to keep in mind that we work for you, and not your insurance company, which means you are ultimately responsible for payment. You may be responsible for any co-payments and deductible amounts or any non-covered services at the end of your visit. Any charges not covered by your insurance, are due and payable at the time of service, unless other arrangements have been made in advance. Any balance not covered by insurance is the patient’s responsibility.

VISION VS. MEDICAL INSURANCE 

Your eyes are part of your body which means if there is a problem that does not fall under a routine exam we may bill your medical insurance. Unlike your medical doctor’s office, we may accept both medical insurance and vision discount plans. You may have one or both. Both types of plans may include co-pays, and deductibles, which are your responsibility, as outlined by your vision or medical provider when you elected your plans. We will answer further questions you may have regarding this.

If your doctor determines you are at risk of an eye disorder or eye disease:
  1. You will be made aware when additional testing and evaluation is necessary to manage your eye health and eye sight.
  2. Any testing for your benefit will be billed to your medical insurance.
  3. Any co-payments or co-insurance are your responsibility at the time of service.
  4. For initial treatment and any prescribed follow-up treatment, your vision plan or medical health insurer determine the co-pay or co-insurance due and are due at the time of service for each visit.
